At Beacon Medical, our primary mission is to provide excellent home-based primary care by using a team based palliative approach in chronic disease management.
This approach empowers our patients to improve their quality of life while enabling them to remain at home and avoid unnecessary hospitalizations.
We provide compassionate, home-based primary care, palliative care & Transitional Care Management (TCM) for debilitated and chronically ill individuals residing in the Pensacola Metropolitan Area. We aim to make medical care more comfortable and convenient for patients by providing comprehensive primary & palliative care in the security of their own homes. With every patient, we craft individualized care plans to enhance quality of life through the aging process.
